You'll need to complete and
submit the appropriate Appealitems for your situation.
Use the checklist on page 1
of the Appeal Instructionsto be sure you've got
everything you need to submita complete Appeal packet.

If you're submitting an
Appeal due to extenuatingcircumstances, it's important
to be clear about whathindered your progress.
For example, medical
issues, a family emergency,or a traumatic experience
along with supporting documentslike medical records, legal
documents, or police reports,are useful in demonstrating
that you facedextenuating circumstances.
In Sections A, B, and
C of the Appeal form,include your
personal information,your reason(s) for appeal,
and the semester you'reappealing for.
Sections D and E are related
to your academic plansat CCSF and your
